Abstract


Viene presentato un elettrocardiogramma (ECG) molto ingannevole, erroneamente interpretato in passato come espressione di fibrillazione atriale. La mancanza di chiare onde P e la variabilità degli intervalli R-R sono le principali caratteristiche alla base di questo equivoco diagnostico. Tuttavia, una meticolosa analisi del tracciato consente di identificare onde F con morfologia e sequenza regolari, indicative di flutter atriale tipico comune. La distribuzione bigemina dei complessi QRS e il rapporto di conduzione atrio-ventricolare (A-V) suggeriscono fortemente, come meccanismo aritmogenetico sottostante, il fenomeno di Wenckebach alternante, dovuto a un duplice livello di blocco nella giunzione A-V.

Abstract


A very misleading electrocardiogram (ECG) is reported which in the past had been mistakenly interpreted as an expression of atrial fibrillation. The lack of obvious P waves and the R-R interval variability are the main features leading to the above diagnostic misunderstanding. However, a careful analysis of the tracing allows the detection of F waves showing regular morphology and timing, consistent with typical common atrial flutter. The bigeminal distribution of the QRS complexes and the atrioventricular (A-V) conduction ratio strongly suggest that the underlying arrhythmogenic mechanism is the alternating Wenckebach phenomenon, caused by a dual-level block within the A-V junction.
