Cardiovascular diseases in pregnancy: when to be aware and what to doPatologie cardiovascolari esacerbate o dovute alla gravidanza: quando cercarle e cosa fare Patrizia Presbitero MD, PHD Humanitas Research Hospital Abstract Alcune delle malattie più importanti che possono influenzare il corso della gravidanza e richiedere interventi sono: la stenosi mitralica, che può peggiorare a causa delle modificazioni emodinamiche che si verificano nel 2° e 3° trimestre. Parole chiave: Stenosi mitralica; Scompenso cardiaco; Aortopatia; Infarto del miocardio. Abstract Some of the most important diseases who can affect the course of pregnancy often requiring interventions are: mitral stenosis, who can worsen because of the hemodynamic modification which occur in the 2nd and 3rd trimester. Percutaneous balloon valvuloplasty in pregnancy is indicated when mitral stenosis is severe with symptoms and pulmonary hypertension. Acute coronary syndrome can also require coronary angiography if ongoing ischemia is present even if the threshold for intervention is high particularly in presence of coronary dissection. Valve prosthesis have to be followed with particular care. Left ventricular disfunction for cardiomiopathy have to be treated in centers where ventricular assisted devices (Impella, Vad) are present. The “Heart Team” (Cardiologist, Gynecologist, Cardiac Surgeons, Anestethist) is very important to provide a good outcome. Key words: Mitral stenosis; Heart failure; Aorthopathy; Myocardial infarction. |
