HDL cholesterol: lights and shadowsIl colesterolo HDL: luci ed ombre Alfio Bianchi Vice Presidente ARCA Lombardia Abstract Il colesterolo HDL (c-HDL) viene comunemente ritenuto un fattore protettivo efficace nel ridurre il rischio cardiovascolare e nel rallentare la progressione aterosclerotica in rapporto ai suoi livelli di concentrazione ematica. Se da una parte una concentrazione ridotta del c-HDL comporta una ridotta efficacia preventiva dall'altra dati attuali confermano che anche valori elevati o molto elevati conferiscono al c-HDL non un aumento della capacità preventiva ma addirittura una possibilità di un incremento del rischio cardiovascolare. Il c-HDL può dunque trasformarsi da fattore protettivo di tipo “funzionale” a fattore di rischio cardiovascolare e quindi a carattere “disfunzionale”. L'infiammazione rappresenta una delle cause maggiori di questa trasformazione. Key words: Colesterolo HDL; Concentrazione; Funzione; Infiammazione. Abstract The high-density lipoprotein is considered an protective factor for the delay of cardiovascular disease progression and for the incident of cardiovascular risk. A lower concentration of HDL cholesterol can be responsible of increase of cardiovascular risk but new data may confirm that high or very-high level of HDL cholesterol may increase cardiovascular risk and reduce the preventive efficacy by the shift from “functional” to “dysfunctional” effect. Many causes are responsible of this shift but the main cause of this harmful effect is the inflammation. Parole chiave: HDL cholesterol; Concentration; Function; Inflammation. |
