Abstract


La corretta gestione della terapia anticoagulante rappresenta un punto fondamentale per la sicurezza dei pazienti dasottoporre ad interventi chirurgici. Il rischio emorragico dovuto al trattamento in corso, va considerato in rapporto al rischio tromboembolico che, a sua volta, non può prescindere dal tipo di intervento da condurre e dalle caratteristiche cliniche del paziente stesso. L'introduzione nella pratica clinica degli anticoagulanti diretti (DOAC) ha semplificato la gestione terapeutica di questi pazienti, relativamente alle modalità e ai tempi di sospensione e di ripresa dell'anticoagulante. Tuttavia è indispensabile l'esatta conoscenza del tipo di DOAC utilizzato e della funzionalità epato-renale di ciascun paziente; occorre, inoltre, che le modalità di gestione della terapia peri-operatoria siano note e condivise da tutti gli operatori sanitari coinvolti.

Proper management of anticoagulant therapy represents a key point for surgical patient safety. The hemorrhagic risk for the ongoing treatment has to be balanced against the thromboembolic risk which, in turn, cannot be separated from the type of intervention and from the patient's clinical evaluation. The introduction of direct anticoagulants (DOAC) into clinical practice has made it easier to manage in these patients manner and timing of discontinuation and resumption of the anticoagulant. Nevertheless exact knowledge of the type of DOAC used and the hepato-renal function of each patient is essential; it is also important that procedures for managing peri-operative therapy are to be known and shared by all the healthcare professionals involved.
