Targets in lipid lowering therapyTargets in terapia ipolipemizzante Natale Daniele Brunetti; Nicola Tarantino; Matteo Di Biase Dipartimento di Scienze Mediche e Chirurgiche, Università di Foggia Abstract La riduzione dei valori del colesterolo è essenziale nella prevenzione degli eventi cardiovascolari nei soggetti con ipercolesterolemia. Il trattamento farmacologico può avvalersi attualmente di una serie di farmaci estremamente efficaci nel ridurre i valori del colesterolo mediante diversi meccanismi (blocco della sintesi epatica, blocco dell'assorbimento intestinale, aumento del metabolismo epatico mediante modulazione del ciclo dei recettori di membrana del colesterolo). Diverse modalità di approccio vengono tuttavia consigliate dalle linee guida delle società scientifiche europee rispetto a quelle americane. In Europa si consiglia di raggiungere target di colesterolo LDL tanto più bassi quanto maggiore il rischio cardiovascolare del soggetto dislipidemico da trattare. Abstract Reducing cholesterol values is essential in preventing cardiovascular events in subjects with hypercholesterolemia. Pharmacological treatment can be based on a number of extremely effective drugs able to reduce cholesterol values, through different mechanisms (blockade of liver synthesis, blockade of intestinal absorption, increased hepatic metabolism by modulation of the cholesterol membrane receptor cycle). Different approaches, however, are recommended by European and American guidelines; for the European Society of Cardiology the best approach is the pursuit of target LDL cholesterol levels; the greater the risk of cardiovascular events, the lower ideal LDL levels (lower than 70 mg/dl for subjects at high risk). By converse, US guidelines recommend the use of more effective statins (high intensity statins) for subjects at high risk. Evidence from randomized clinical trials with inhibitors of intestinal cholesterol absorption and PCSK9 antibodies challenged the target of 70 mg/dl, showing further reductions of cardiovascular events with even lower LDL value (55 mg/dl for ezetimibe, 30 mg/dl for evolocumab).
