The apparent paradox of beta blockade in heart failure. A dive into the past ... always currentL'apparente paradosso del betablocco nell'insufficienza cardiaca. Abstract Left ventricular systolic impairment has been considered for many years the centre of the cardiac heart failure syndrome (CHF) thus the target of therapeutic efforts has been the increase of systolic pump. The negative prognostic impact of almost all drugs with positive inotropic effect and the negative prognostic value of the indexes of sympathetic drive, suggested that neurohumoral system would have a key role in CHF. The counterintuitive use of drugs with negative inotropic effect such betablockers, was successful in several historical clinical trials and betablockers became a cornestone in the treatment of CHF. Carvedilol, Metoprolol CR/XL and Bisoprolol were tested in these trials. The gradual increase of doses and the achievement of heart rate between 50 and 60 beats per minute when the sinus rhythm is present, are recommended. Atrial fibrillation, advanced age, diabete mellitus, chronic obstructive pulmonary disease do not represent a contraindication to the use of betablockers. Observational data suggest that betablockers could confer advantages also in acute CHF, in case of need for inotropic drugs, and in presence of aortic stenosis but these conditions deserve particular care and clinical judgement.
