Dati di registro indicano come il 5-10% dei pazienti affetti da insufficienza cardiaca si trovino in uno stadio avanzato di malattia, con una mortalità annuale superiore al 50% ed un'evidente discrepanza tra riceventi e donatori. Ciò evidenzia la necessità di un'attenta e precoce selezione dei candidati al trapianto da parte di un team multidisciplinare, che identifichi i fattori di rischio reversibili, valuti periodicamente il paziente elegibile e tenga conto della natura dinamica della lista trapianto. Abstract Heart transplantation is the treatment of choice for selected patients with end-stage heart failure. In fact, 5%-10% of heart failure patients are in advanced disease stage with 1-year mortality in excess of 50% and poor life quality. Unfortunately, constant shortage of donor hearts in front of the increasing prevalence of patients in need of heart transplant generates a marked discrepancy between supply and demand. This aspect, which underscores the importance of a stringent candidate selection, has led to generate guidelines for heart transplant, pointing out the importance of early evaluation by a multidisciplinary team, the need to address reversible cardiovascular risk factors and the need of frequent reevaluation of eligible patients, taking into account the dynamic nature of transplant lists.
