Vito Maurizio Parato, MD UOC Cardiologia, Ospedale Madonna del Soccorso, San Benedetto del Tronto, Italy Abstract Le linee-guida internazionali classificano l'infarto miocardico acuto sulla base della presentazione elettrocardiografica in STEMI (ST-Elevation Myocardial Infarction) e NSTEMI (Non-ST-Elevation Myocardial Infarction). La transmuralità, da sempre indicata come carattere differenziale dello STEMI, non sembra risparmiare del tutto i casi di NSTEMI. 11In questo articolo riportiamo scenari clinici in cui il passaggio da NSTEMI a STEMI nello stesso paziente può portare a complicanze gravi, talora anche mortali. Abstract The international classification of acute myocardial infarction is based on ST modification at 12-leads EKG: STEMI (STElevation Myocardial Infarction) and NSTEMI (Non-ST-Elevation Myocardial Infarction). We know that the transmurality characterizes STEMI but also NSTEMI may be involved by a transmural development. In this article, we report some clinical scenarios in which the shift from NSTEMI to STEMI in the same patient may lead to severe and life-threatening complications.
