Abstract Coronary revascularization by percutaneous coronary interventions with stenting (PCI) and coronary artery bypass graft (CABG), represents a cornerstone in the treatment and stabilization of patient with symptomatic coronary artery disease. However, interventional procedures do not stop atherosclerotic disease progression and do not finally prevent life-long adverse event re-occurrence. Scientific international guidelines are lacking on how and when performing diagnostic stress testing in asymptomatic patients after coronary revascularization procedures, with the aim of silent ischemia assessment. Furthermore, the evidence of limited extension silent ischemia, and subsequent revascularization procedures, do not seem to modify the prognosis of asymptomatic patient without major risk factors associated. Recommendations about appropriateness of cardiovascular imaging state that echocardiography or nuclear stress test performed before 2 years from PCI and 5 year from CABG is inappropriate in asymptomatic stable patient, with complete revascularization. However, patients with major risk factors, like diabetes requiring insulin-treatment, often leading to unexpected adverse event, have to be tested in a shorter time, even in absence of symptoms. Similarly, patients with incomplete revascularization must undergo an early, but not urgent, evaluation, depending on persistent lesion characteristic and patient risk profile. Functional imaging stress test provide a high diagnostic accuracy after revascularization, and have a main role in the decision making process, when compared with anatomic evaluation and ECG-dependent testing, according to the evidence that site and extension of induced ischemia have an important prognostic impact in asymptomatic patient.
