Vincenzo Carbone 1, Verdiana De Caro 2, Stefania Criscuolo 3, Giovanni De Caro 4 Abstract Abstract In the presence of a wide QRS complex tachycardia, the differential diagnosis between ventricular and supraventricular tachycardia with aberrant conduction or pre-existing bundle branch block is mandatory. Indeed, the two forms have an arrhythmogenic substrate, a prognostic impact and treatment strategies substantially different. The Holter monitoring of a 54-year-old patient presenting with episodes of palpitations is reported here. Three-channel ECG recording showed a nonsustained, wide QRS complex tachycardia. Through critical analysis of classical differential criteria, but mainly through the identification of sometimes modest electrocardiographic signs, the clinical case illustrates the diagnostic workup of wide QRS complex tachycardias.
